Perinatal Mood Disorders: Implementing the Edinburgh Postnatal Depression Scale Screening for Antepartum Patients with Maternal-Fetal Complications

Abstract
Background: Perinatal depression, (PD), encompasses both prenatal and postnatal
depression, significantly impacting maternal health and infant development. Early
detection through screening can lead to timely interventions. Women that are admitted
to inpatient antepartum services are being removed from routine outpatient prenatal
care. As a result, perinatal mood disorders are not being screened for in these individuals
which puts them at risk for undiagnosed perinatal depression.
Objective: This manuscript aims to evaluate the effectiveness of perinatal depression
screening in inpatient settings, exploring current practices, barriers, and
recommendations for improvement.
Study Selection: A systematic review of literature was conducted using databases such as
PubMed, CINAHL, Scopus, Web of Science, PsycINFO, and Cochrane Library to identify
studies published. Key terms included “perinatal depression,” “screening,” “inpatient care,”
and “interventions.” Data on screening tools, outcomes, and implementation strategies
were extracted and analyzed.
Results: The review indicated that while screening tools such as the Edinburgh Postnatal
Depression Scale (EPDS) are effective, only a minority of inpatient settings routinely
implement them. Barriers to screening included lack of training, insufficient time, and
stigma. Successful interventions demonstrated improved detection rates and maternal
outcomes when integrated into routine inpatient care.
Conclusion: To enhance perinatal mental health outcomes, it is crucial to implement
systematic screening protocols in inpatient settings. Addressing barriers through staff
training and policy development can facilitate the integration of screening and support for
at-risk mothers. Future research should focus on long-term impacts of screening on
maternal and child health.
