Abstract
The escalating pediatric behavioral health crisis has heightened the demand for integrated behavioral health (IBH) within primary care settings. The Virginia Center for Health Innovation’s Primary Pathways pilot sought to expand IBH access; however, early implementation identified significant variation in care manager role execution. In collaboration with a Doctor of Nursing Practice (DNP) team, a standardized care management toolkit was developed to address gaps in clinical knowledge, communication, workflows, and care coordination. Establishing standardized care manager competencies and workflows is critical for enhancing pediatric behavioral health access, improving outcomes, and ensuring the sustainability of IBH in primary care.
