Abstract
Doctor of Nursing Practice (DNP) scholarly practice projects often vary in rigor, relevance, and sustainability. This paper describes an extended academic–practice partnership between a university and a nonprofit, multi-stakeholder organization leading a statewide pediatric behavioral health integration initiative. Using the Comprehensive Health Integration and RE-AIM frameworks, DNP students collaborated with interdisciplinary partners to assess care management workflows and co-develop a scalable, practice-ready care management toolkit. Extending academic–practice partnerships beyond traditional clinical sites enables DNP work to advance system-level implementation priorities and foster sustainable practice change.
